A term male born at 38 weeks had difficulty feeding on day of life one. Newborn exam demonstrated heart sounds heard on the right. A chest x ray revealed dextrocardia and abdominal situs inversus. Transthoracic echo and cardiac CT demonstrated dextrocardia, type A interrupted aortic arch, type II aortopulmonary window, and secundum ASD vs PFO. On day of life eight, the patient underwent aortic arch reconstruction under deep hypothermic circulatory arrest. Postoperative echocardiogram shows successful repair of the aortic arch with no residual AP window and normal ventricular function. At 10 months follow up, the patient is gaining weight appropriately and completing all developmental milestones.
